Level 2 Background Screening

The following information is for those applying for Level 2 Volunteer status.

*CODE REQUIRED FOR FINGERPRINTING - FPCPSVOLUNTEERS

Please follow all steps in the application process to ensure that your fingerprints are sent into the Agency for Health Care Administration (AHCA) Clearinghouse.

If you conduct the fingerprinting process outside of Fieldprint (such as with the Polk County Sheriff's Office) or not including the correct PCPS Fieldprint code, this may result in your fingerprints not being sent to the Clearinghouse.

This could result in you having to be re-screened and paying for another screening.

The Volunteer Office of Polk County Public Schools does not manage the program or screening process for private and charter institutions. If you wish to be screened to volunteer at a charter or private school, please reach out directly to that school to get more information about their process.

Level 2 Application Process

Step 1 - Complete the online Level 2 Volunteer interest form: https://forms.office.com/r/0JhQT5QXHn.

Step 2 - Create an account and set up an appointment with FieldPrint for fingerprinting. When making the fingerprinting appointment, enter code FPCPSVOLUNTEERS using the following link: https://www.fieldprintflorida.com/

** If you believe that you already have fingerprints in the AHCA Clearinghouse, only complete step 1; PCPS will then conduct an agency review with AHCA to use you existing fingerprints.

You will be notified by PCPS once your screening has been completed.

Prospective applicants will be screened by the Level 2 screening standards outlined in Florida Statute 435, which list the disqualifying offenses that will result in a denial of the application, including any conviction of, or adjudication withheld for a felony offense.

Click here to review disqualifying offenses, or search "Florida Statute 435".

If you believe that you would be exempt from having to be background screened for volunteering, please fill out the form here: https://forms.office.com/r/ztESE0Cvrz.
The following groups could be exempt:
- Current PCPS Employee
- Contractor or Vendor with PCPS with valid badge
- Active Kelly Substitute
- Active Law Enforcement officer with arresting power
